What does this do on Recordings tab on the settings dialog? Also, is it EPG dependent?

Very much so. I think the US (possibly North America?) has a unique field in their EPG for each show and episode. with this and keeping old entries in the database, (even when a show has been deleted?) enables it NPVR to ignore future recordings with this identifier.
This in the past has caused some support questions around "This show didn't record".

Sadly, the rest of the world doesn't seem to have an EPG source with unique identidiers.

that it may be further restricted to Schedules Direct EPG (if currently implemented????)

double check - yes...

2.0.3 available (1st March)
- added support for schedules direct unique identifier for avoiding duplicates
