I installed MST and after several attempts, it now seems to be working. I am not sure why. It is still on XP, so powercfg would not be an option. Hopefully, it keeps working.

Really appreciate this comment. I've had loads of problems getting the sleep functions to work properly on one of my machines and have searched a number of times for answers without ever running across this command. It is not working again. It does seem to be NPVR that is causing the issue. If I exit NPVR, then it will go to sleep after the idle period. However, if NPVR is running, it will not.

I'm doubtful it will work but try turning off the NPVR screen saver and see if that helps.
When it wakes, are you running the powercfg command to see what is waking the computer or it will not even enter sleep at all with NPVR running?

I tried turning off the NPVR screen saver with no luck. It doesn't enter sleep at all when NPVR is running.

What's your background CPU usage when it's running and sitting doing nothing in the menu?

It now seems to be working. I am not sure why, but it has gone to sleep on its own a few times now with NPVR running. Hopefully, it keeps working.
