I think you are seeing XMLTVExtras and TVListing working for you with the icon source, let's leave it at that. SearchLite, is totally different, but it can add cover art to its' Recording popup too and it doesn't need the icon source.

The utility XmltvExtras also adds additional information for programme images automatically if your xmltv-source includes them. Alternatively or in combination with the images generated by XmltvExtras, the plugin looks in the "Media\Shows" directory for programme-images. See Configuration below for details. Images from TheTVDB are used automatically for linked shows.

Sub and Iain, pertaining to what I call dynamic background problem how about a happy medium? I propose that you just show the thumbnail as it's done now but once you press "ok" or "enter" on the folder how about a quick check to see how many files are inside. If there is only one or a video_ts folder then have a pop-up that shows fanart.jpeg and have "play" and "back" buttons available. It'll be something similar to how the recording plugin works when you select a file. If, however, there are multiple media files inside then go into the directory as normal.

This would satisfy me without the need of a database.

EDIT: the pop up will be fullscreen with the buttons overlayed on top
