Ok to confirm your Orbi router is directly connected to the internet and not to a modem or router from the ISP.

Big update! I contacted my ISP and they said this: 

Well by default, we do not do any particular port blocking. Port forwarding is done through the router typically. Sometimes what fixes this issue is putting your router on a public IP address. We could try that just to make sure that it isn't being blocked by us

After they did that...IT WORKS!

Ok great of course you need public internet for public access and not CGNAT or things like that.

Be aware, you've exposed the device NextPVR is running on to the Internet.  Typically, you may want to put a reverse proxy in front of that to limit the exposure just to the proxy device. 
You may want to consider:

- Only enable port forwarding when you need it. Keep it disabled otherwise.
- Put a reverse proxy (I use nginx on a raspberry pi), as the device the port forwarding goes to.
- Setup a VPN

I agree but the plan is to expose NextPVR to Emby on the seedbox so he should be able to create a secure tunnel to his mac.
