I'm still using 4.2.6 and everything is working fine. Should I upgrade to 5 or 6? Can I install 5 as an update or do I have to uninstall 4 first and start over?

First decide why you want to upgrade. It is an new program and it will not overwrite v4 so it is pretty easy to evaluate, especially if you are willing to start over. You can import recordings if needed.

Ignore v5 as only v6 is supported now. Regarding what you can and can't migrate really depends on you tuners. I'd follow the unsupported steps on the wiki to see if the simple database and config copy from v4 works for you. If it doesn't you have a bit more work.
