A possible new theory...
I tried adding the divide by zero patch (no 4 on the sticky), but presuming that tsreader was causing the problems, only replaced unmanaged.dll. The bug with the dropped playback of recordings immediately returned. I've now also patched the tsreader file, and touch wood, I haven't had any dropped recordings so far (this is again only with an hour or so of playback so far).
Could it be that NPVRtsreader2.ax, and unmanaged.dll need to be a 'matched pair', and that my problems are caused by ending up with an unmanaged.dll from one patch, and a NPVRTSreader from another patch?
If this sounds a reasonable hypothesis, could you post the most recent versions of these files together so I can be confident that I'm using the most up-to-date versions.

Unfortunately my version of unmanaged.dll is no longer compatible with 2.4.x, so I cant supply the latest for you to test that theory.
