Ok - that narrows it down a lot. Now I know approximately where your problem is happening.

It looks like maybe it's having an issue querying the signal quality.

Looks great - Icon reflects Live TV & both Tray Status & Web Status show the LiveTV active.

Great. It looks like whatever tuner you’re using doesn’t support the BDA driver interface to query the signal quality, and NextPVR wasn’t handling this very well.

Also have a "Winfast DTV Dongle Gold" - not tried on NPVR V5 yet.
