The file wasn't locked by being played back. It just occurred to me that it's possible my Plex server might be locking files as it triggers a metadata update when it sees an updated file - but why would it lock a file when it's only reading it?
I've tried re-running iGL as you suggested but with the same result, as far as I can see. I've updated the logs and database with more files for you to take a look at - same URL.
PS: The rename to E66 is a pain, TBH, as it means I have to manually rename files to stop Plex from assuming there are 66 episodes...
