To give us a bit more info, create a c:\temp directory, and set HKEY_LOCAL_MACHINE \Software\Wow6432Node\NPVR\LogTSWriter=1 (DWORD) in the registry, then repeat the process, and post those same logs, and the new c:\temp\writer-x.txt file.

The expected log messages are there now.

It looks like your problem is related to this:

Ok just tried that real quick, logs and writer file attached. Will try more tomorrow, once again thanks for the help.

Sorry, you're going to need to update your NPVRTSMon.ax with this one, and post those logs again. I've added some extra logging around the mutex creation.
