My suggestion if you are trying to debug NextPVR playback is restart the recording service and avoid live tv from xbmc. To me it looks like channel change bug is creeping in make things much harder to triage.
I notice that both signal strength and quality is very low, and no file is created. Don't know if that is because the device does not deliver any data.

I've had no problems with signal strength or quality in the last 6 years that I've been using this aerial setup. (Roof aerial pointing at Crystal Palace).
All SD channels are currently working without issue.

Not sure it is relevant for you, but HD channels operate at a much higher bitrate and thus require a much more reliable/stronger signal. Do you have any splitters that you could try to remove or is it possible to shorten the cable length as a test?

My experience has been that signal quality issues can cause scanning problems but then work okay for recording. If you have scanned okay then it should record okay.
