We do indeed have a proper Freeview aerial installed.
Again - the problem is not reception; a different application works just fine. (But its scheduler is crappy, so I'd like to use NPVR.)

I'm practically on top of the relay west of Cambridge though.

That tool reports strength minimum; but reception quality 100% for that frequency. (Bandwidth 8MHz)

Checking... other channels on that mux are:
Al Jazeera English (fails in NPVR)
Yesterday (fails in NPVR)
... yeah; looks like it's the whole of that multiplex failing. Which makes sense.

I'm baffled. This describes capturing logs from wintv7 ... http://www.hauppauge.co.uk/forum/showthr...-my-Thread

You could try doing a scan in WinTV and then a scan in NextPVR and posting the logs from both apps here. Something might stand out.

Installed updated NextPVR, and things are now slightly better.
I no longer have some channels which appear in the list but don't work - now they don't appear in the list at all. Specifically, ITV3 is missing. Some others probably.
It doesn't help that the numbering bears no resemblence to thew usual freeview numbering.
A couple of the previously missing ones DO now work however.

I've got logs from both wintv and nextpvr. NextPVR complains a couple of times with UI asserts - I think that's it complaining about not finding my plugins any more (since I intended to install fresh, but seem to have missed a config file somewhere.)

NextPVR isn't complaining as much as TV Listing. Why not just remove it and try the default guide?

Yes. The guide plugin complains. I ignored that, and used the main one. Several channels are not listed.
Looking at the log - I can see it looks like it's failed to get signal from some of them:

2013-06-28 21:41:11.260 [DEBUG][1] Calling LockChannel()

But the channels are there, and do work. (in WinTV)

And, indeed - it seems to have picked them up in the metadata:

But I can't see them in the list - either in programme guide or in the "Channels" live tv list.

(A possible bug - if I start typing into the search field of that channel list, as soon as I type the first character it immediately switches to the first channel that matches, even when there's more than one match - I'd expect to see a list)

And now something seems to have sorted itself out; I have ITV3 again (although it's on an odd number; I can see where to fix that)
