According your LOG, you are not using PDVD decoder from Unlocked SAF, you are using "CyberLink H.264/AVC Decoder".
The one SAF is using is named "CyberLink Video Decoder (PDVD13)".
So try set correct one (best way is to uninstall software that installed CyberLink H.264/AVC Decoder, (it can be TV card software or old PowerDVD player, etc...), then reinstall unlocked SAF and set correct PDVD decoder in nPVR setup (if the wrong one is set).

Ok, I removed my PowerDVD software and installed SAF6 properly and tried it with the proper codec as mentioned.

Still lagging audio that gets worse as you go.

I'm pretty stumped.

Can you try another test? Can you try record station where you are getting syns issue.

Then exit nPVR. Go START-SAF-Tools-Graphstudio, drag and drop your recorded file here and hit play.

How it play?

BTW, what GPU/CPU do you have?

Fixed!!! I figured it out.

Did some research and found out others using SageTV were having the same problem with the Hauppauge HD PVR. They rolled back the driver one revision and it worked.

I rolled mine back, and it seems to work fine now. Tested it a few times, no lag at all.

I feel bad that it wasn't an NPVR problem the whole time, but you just never know what it could be with these things.

Only remaining problem is that Live TV only gives me a blank screen. I can hear the audio but not see anything.

Thanks you guys for all your help on this, it did help to exhaust everything. To boot, I can play more DVD's with the new codecs.

In Settings -> Misc2, change the setting for "Use alternate OSD for missing video" (or something like that).

At some point yesterday I started to realize this was a recording problem and not a playback problem, but didn't know how that could be or what to suggest. Glad you finally found the cause.

Live TV works great now. System is flawless once more.

johnsonx42, yes that is how I finally figured it out, I thought that it had to be a recording problem with the HD PVR. I tested it in Total Media Extreme, and it was lagging too!
