I am using the NextPVR client and the LAV Filters AC3 decoder consistently loses sync between the audio and video for me on my system and has been an issue for years. I found AC3Filter 2.2a which is an older audio decoder that works great. The audio and video stay in sync when I use it. 

If anyone wants to try it out, Here comes the WARNING! The newer version on the old AC3Filter.net project website has been hacked and the download has a virus. So BE CAREFUL where you download it from!!!

Honestly, I’d just go LAV decoders unless there is something specific you need from AC3Filter.

I tried, but for some reason the audio and video would not stay synchronized no matter how much I messed with the settings.

That's weird, because it's a widely used decoder that performs very well.

It could be your configured audio renderer or audio device causing sync issues. These components are responsible for the clocks and timing.

I think the majority of my issues is that i'm 40 to 50 miles from the broadcast towers and I get plenty of signal interference due to weather, solar activity and the like. I have two 60 mile antenna's and a signal booster and still get plenty of interference.
