I was having sound problems with Live TV, or at least watching from guide listing -- is that the same thing? Sound would stutter, be WAY out of sync or completely silent if "pause + play" cure was used.

I decide to uninstall ffdshow and Haali, which were pretty old, and find new versions. Then I got a new version of ffdshow (called ffdshow tryout) and sound problems disappeared using ffdshow for all audio, including AC3.

But I also then tried an MKV file, and it would not play, of course. So I installed latest version of Haali, and MKVs would play, but sound problems on Live TV returned. Uninstalled Haali and sound problems remained. Uninstalled ffdshow and reinstalled and sound problems fixed again. It's as if Haali messed around with ffdshow or system settings somewhere.

FWIW, I noticed when sound was not working right, that playing a video, the start process was "smooth." But when "watch" was used from the Guide, NPVR did a "double clutch" (showing my age there!). It would start "immediately" and then stop for 1/4 second or so, start the video again and then the sound a short while later. Now, with the sound working well, it starts smoothly, just like videos do.

Right now ffdshow is handling AC3, but I haven't configured it to output the way I want yet, but I did specify 5.1 during the install. I haven't verified it's actually outputting 5.1.

I've had the Haali spliiter installed with ffdshow (tryout) for mpeg video including h264 and ac3/mpeg audio for a few years without any major problem in live tv. I do now use this config.xml option for best results

Code:
<ForcedLiveTVClock>AC3 Audio Renderer</ForcedLiveTVClock>

I only have a few MKVs so I'm converting them to MP4 to avoid the extra complexity of playback. I'm so glad to be rid of the sound problems that I don't want to rock the boat.

FWIW, the Combined Community Codec Pack http://www.cccp-project.net/ is what I use.
