CableLabs. Period. They are the one and only outfit that can certify a product for use on cable systems. Upfront cost, last I read, runs around $150,000 US for them to certify a solution. THEN you get to pay somewhere around $100,000 per year for the license required by them. The cable companies have done everything that they possibly can to make using a cable card as painful as possible. Why? Just in the US they make around $20 Billion per year on cable box and DVR rental fees. They really don't want us to use cable cards at all, but they still have a firm grip on the DRM aspect.
