The reason that Close is at the top of most lists is to protect against unintentional (double) keystrokes - so if you accidentally press the OK button twice on your remote, you simply return to the TV Guide - no damage done.
In general, it's good programming practice to make the default action Close or Exit.

It's unlikely I'd change it now, since it's been that way many years now, since the beginning of NextPVR. Also, I think this is going to be one of those things you can never get everyone to agree on.
Close was intentionally placed at the top as the default, so it was harder to accidentally perform some of the other actions. Some of the client interfaces, like MVP/NMT, were pretty slow and it was was easy to accidentially push a button twice out of frustration.
In a future build I might be able to provide an option for this, for users that prefer an alternate order of these buttons.

It's not quite as simple as that...
So you press the OK button to select a show - the NMT UI is slow to respond so you think that you may not have pressed it hard enough - you press it again - the UI catches up and you're now watching or recording depending on which option is at the top and it's pretty certain that wouldn't be the option you wanted...
