Login changing on its own

I never changed the login from default. But for the past 3 months or so, every time I log in, it doesn't work the first time or the first few times I try it. I discovered that the login credentials are somehow changing without any prompt to do so. Sometimes it requires to check "remember me" and sometimes not. Sometimes it requires user name and password, and sometimes it requires blank. I finally get logged in, but it takes awhile and is a very annoying condition. It seems to randomly require a certain combination of credentials, and so far I get logged in only after I've found the particular combination by trial and error. Any one of these might get me in:
User Name: admin  Password: password  Remember Me: unchecked
User Name: admin  Password: password  Remember Me: checked
User Name:            Password:                Remember Me: checked
User Name:            Password:                Remember Me: unchecked
One of these combinations will get me in, but only by trial and error.

I've tried restarting the service and rebooting, but the problem is persistent. Is this a known issue?

Maybe you are leaving the page open in the browser, it is best to exit and restart. No errors are being logged in those 2 days of logs so it might be your browser's cache. Also check to make sure javascript is enabled.

It is better to choose to use either your browser's auto fill or the permanent login but not both, again clear the cached and NextPVR's storage to get things clean.

I don't leave the page open. Every time I exit the browser, it deletes cache and history. I'm using Edge, and javascript is "allowed". It's also a Win 11 Home OS.

I don't know what you mean by "permanent login." I left it at default.
I don't know what you mean by "clear the cached and NextPVR's storage."

Maybe you don't always exist, do you let your PC sleep with the browser open? As I said there were no errors in the logs.

The permanent login is "Remember me". You shouldn't be ticking and unticking it. The advantage to using it is you don't have to start on the login page.

Browser have caches not just of cookies. Ctrl-F5 doesn't clear the storage when "Remember me" is ticked

I've not seen any other reports of this.

Web browser's (or password apps with browser plugins) can be pretty insistent about remembering username/password fields. It's possible your browser is trying to automatically set these fields when you go to the settings page?
