The Alt-Enter sure changes the mode, but it screws up the position of the window. Besides it doesn't go back to fullscreen correctly. If I use fullscreen and then hit Alt-Enter, it goes into windowed mode, but it maintains the same size, as when it was fullscreen, and if I try to go back into fullscreen mode, the titlebar suddenly won't go away.

Are you running the current v0.97.13 release? I was aware of a bug like this in earlier releases, but this should be resolved in the current release.

Hate to drudge up an old topic.. but after I installed the last beta Nvidia drivers I've got a window border down the left edge of my screen.

I think it's fixed. I just alt+entered a bunch of times, opened the config and changed settings back and forth (no overall change, just to make it save and restart the service) and I'm now running at windowed, but a tad bigger than the screen.
