On a lot of systems, the DVB-EPG doesnt reflect last minute changes, and you often see it show a program name that isn't showing on the screen.

I'd agree with that. From what I have seen, time changes of +/- 10 minutes or so do not appear on the EPG's at all for the same day, especially for things like sport or concerts that over-run rather than just re-scheduled and for things like the RadioTimes xmltv listings you are lucky if they are updated weekly.
