I apologize if I mislead anyone, but I did not mention that it was unencrypted qam (Clear QAM) in my first message, it does work just like OTA.
If using OTA will mean I can use just XBMC+NPVR, I really don't mind going OTA since I am not getting any additional channels on QAM, it is just clearer.

I can also try adding channels manually to NVPR if that's an option by copying their c-qam numbers off HDHR or WMC, if this can help verify anything.

First, in case it hasn't been made clear before, in NPVR Setup you need to choose the devices labeled "QAM", NOT the devices labeled "HDHR". The HDHR devices are actually for HDHR Prime and DCR-2650 devices using cablecard. For some reason NPVR can't tell the difference between the two types of devices.

Second, if you can't get any scan results using the QAM device, try changing the Application setting in the HDHR driver setup to "Other ATSC/QAM" instead of NextPVR. Some users have reported that they couldn't scan with it set to NextPVR, yet then had to change it back in order to use the app once the scanning was done. This doesn't happen to everyone, and the reason remains a mystery (as far as I know anyway).

If it doesn't work after that, post logs again.

OK, I reset the HDHR and selected "Other" again, and scanned QAM not HDHR (totally missed that it said "Prime") and got a huge list of channels, NPVR selected unencrypted ones automatically, and I can confirm those are the places matching the CQAM channels when I use my TV to Scan, but the odd thing is while all encrypted channels show numbers, clear ones show up as blank, so I would have 44.1, 44.2, 44.3 encrypted [ BLANK ] unencrypted, 44.5 encrypted, and so on.. Some of them do have numbers, though. For the ones missing numbers, I will just go by Frequency and SID# but when the rest finally disappear, in Channel list there are no freq# to match.

I also signed up for schedules direct, but to my surprise they dont list QAM option like HDHR's site does.

What should be my next step after channels come up? I just hit next, saved it all, matched just a few channels to try, I am not getting any channel to work. It keeps saying "the tuner you were using was required to record, would you like to switch to recordin, or exit back to the menu?"
When I select a channel that is mapped like WNBC_HD to something, I hear the sound for a split seconds, then I get above message..

EDIT: I played around with a few things and changed it from ffmpeg to Microsoft DTV-DVD and it seems to work now, but I will test the channels tomorrow! Excited!!

The guide source doesn't seem to work, is there any other way I can get Guide data?

With NextPVR you can't use channel numbers such as "44.4" but you could input "444" or any other whole number that suits you under Settings -> Channels -> (select a channel) -> Details.
[Edit] You can still find the freq# under Settings -> Devices -> (select a device) -> Device Setup.

That "tuner you were using was required for a recording" message is worrisome, but has it ceased since you selected the Microsoft DTV-DVD Video Decoder for MPEG2 Video? When you say "it seems to work now," do you mean that you're seeing video in live TV? Digital channels in the U.S. almost always use AC3 Audio, or rarely MPEG1. You can't use the Microsoft decoder for AC3, so you might need to download something like AC3Filter (http://www.ac3filter.net/), which is what I'm using.

SchedulesDirect should certainly work if your channels are mapped. Another option discussed in the Wiki is mc2xml, but setup is more complicated.

Mapping QAM is a nightmare when there is no channel information (none of mine do). You end of having to load all the unencrypted channels and then watch them to decide what the channel is and then you go through the 10 clicks it takes to map the channel to the EPG data. When there was QAM here it took hours.

"it seems to work now" is based on random test, I just picked WABC and it works, but when I go to LiveTV and watching WABC or WNBC, hitting up takes a while to load the next channel.
By the way SchedulesDirect worked (I manually mapped channel xxx.xxx to 7 WABC and I am seeing the guide in LiveTV now) Changing channels is still very slow, sometimes I have to stare at a black screen for a good while before choosing Main Menu because the channel is not coming up. Then I try another channel it works. I guess I will have to test the channels one by one.

Martin, I spent hours if not DAYS setting up QAM on my WMC. Keep in mind WMC does not recognize any channels at all, you cannot search for them, basically NPVR is still much better at least it can find all clear channels using a simple search. With WMC I had to rely on HDHR's search results, go back to WMC, add manually, then look it up and link to guide info, rename and renumber, took forever. Luckily NPVR at least found all channels, all we have to do is just link them up with SD.

Since I am using Basic Cable QAM, my channels are limited, I will just use the big package in SchedulesDirect and then choose my channels, because the Basic Channel package on SD is missing some of my basic channels.

Next step is trying to get this to run on XMBC. Can I use the XBMC for guide set up?
I am now going by Pinned Channel list, on eby one, and then writing them down on a pad 115 - Food Network HD, and once I have them all, I will remove the ones I don't care and link up the ones with SchedulesDirect. Is there a better way to do this while watching and linking? I can select all channels on SD and then just link the ones I have in one shot.

Unfortunately they're not required to transmit channel metadata on digital cable networks, and they usually don't so NextPVR has no idea what the channel's name or number is. Pretty much all you can do is watch the channel the channel to find out what it is, then right-click to get the context menu, and select 'channel epg source' (or similar - I'm not in front of my NextPVR PC right now to check).

Thanks Sub,
I was able to get all my channels and deleted all unnecessary channels. I am having trouble with just one frequency and 4 ID's within that frequency, they never show up in the searches and Martin helped me add it manually, but even though channel showed up fine yesterday it is not working.
