If you run "nextpvr.exe -no3d" does it make any difference?

Wow, good suggestion, it took some digging to find the Application logs but here is the latest, which shows the Faulting module name, ntdll.dll. Is that a Windows .dll?

I also tried uninstalling my Kworld software for my TV tuner USB stick device. That did not help either.

Here is an updated nextpvr.exe, with more logging, which should help us narrow down the cause of the problem.

Sorry, ran that and it did not help. Still the same startup error. And there is not a new .log file showing, like not recording a log.

Like johnsonx42 I suggest it might be Windows itself. Maybe from and admin command prompt type

sfc /scannow

Another thing about Windows 7/8 = even though the user settings show me logged in as administrator (I'm the only user on this computer), when I go to the command prompt and enter sfc, it says I need to be logged in as an administrator. Somebody at MS needs their head examined.

You need to right click on the command icon and select run as administrator. It's to prevent malicious code from running on your pc.

You're absolutely sure you installed all the pre-requisites listed on the wiki (post #4)

Microsoft .NET Runtime v2.0.
Windows Installer v3.1 or later.
DirectX 9.0c runtimes. (Not sure this is required on windows 8)
Microsoft Visual C++ 2008 Runtimes.
