Startup in live tv

When I click on the pvr icon I'd like it to open and go to live tv of the last channel I was watching when I last closed it (not to the main menu). Possible?

(Well, the mode where if I click the right mouse button it goes back to the tv guide listings, maybe that isn't live tv, but the tv guide mode but watching a certain channel)

Right click your icon and add this to the target
-direct "live tv"

But if I right click I have to go back to the main menu, then into the TV Guide.

When I "Watch Now" from a channel selected in the Tv Guide, I can then right click and go immediately back to the tv guide without going through the main menu.

How do I start in this mode?

BTW, is there a way to "Watch now" when I double click a show in the tv guide? (instead of going through the pop up and selecting "watch now" button?)

You cant do exactly that, but you can press the play button on your remote (Ctrl-P on keyboard) with a currently playing show selected to start watching it without seeing the popup.
