Just noticed this post - you should have posted in the support thread.
First I would do a cold reboot - ie plug out of the wall for 5 mins - sometimes (like about every 6 months) my motherboard throws usb woopsies with the pctv HD nanostick that only that will fix.
Don't install any of the software that came on the CD. Then install all the patches in the sticky in date order - at least your system will be at the latest available state and the last patch could be helpful.
Codecs - I would PM Hoborg for the Unlocked SAF6 - the PDVD13 codec can't be beaten for H264 or MPEG2 on Freeview. Audio - I would use LAV for MPEG1 and HE-AAC, you may also need AAC (If the audio has been re-encoded on any of your recordings) If you have Satellite you will need AC3 as well. Otherwise disable along with E-AC3. Renderers you have correct. Don't forget these settings are for Live and Recorded TV only. (I use LAV video with copy-back acceleration on the DVD tab and for MP4 MKV etc. set via filter priorities - with xy-vobsub filter for Subtitles - works well)
If you still have problems - usually picture break-up - you need to try a powered hub for the nanostick - the tuner chip on the original ones was a Sony TV Tuner chip and is very power hungry - It took me 3 months and a new aerial and amplifier to find that out
I'm not sure which transmitter you are on but they recon that:- In London, in particular, Freeview received from the Crystal Palace transmitter is unlikely to be affected by mast activation because of the strong terrestrial television signal and its relatively large frequency separation from 4G at 800 MHz.
Also check that the tuning ini for the transmitter you are using is up to date - there have been several re-tunes this year on some transmitters.
I am using Windows 8.1 with the PCTV HD nanostick using the Windows found driver and NPVR with almost no problems - so it does work when set up proper.

It's not an overly complicated system - it's more - overly simple operatives
