Tried other browsers: Windows/Chrome, Windows/Edge, Windows/IE, Windows/Firefox, Linux/Firefox, Linux/Chromium, same result, a dialog that says page says "undefined".

I attached a couple snips that I picked up on the way, where the UI is messed up. Especially IE (does anybody use IE any more?), it's totally non-functional.

Pretty sure that's the right channel lineup but I can try others for diagnostics.

Yes, it is CableCard.

Tried other lineup:
"Frontier FIOS - Cable" - also undefined

So I tried "Schedules Direct Custom" (see screenshot), and it seemed to associate the channels and claimed to download data, but no EPG data (see screenshot). I didn't really expect that to be right, but maybe successful, and maybe closer. Well, it didn't say "undefined" so maybe that's progress.

I tried a few other random choices of lineups - to see, maybe it would succeed in getting a guide, though all the guide data would be wrong. I didn't find any that reached the point of actual EPG data. Several of them gave "undefined" dialog and several others just fell back without notice to the starting screen where it says, "the device has no channels". Other than the CUSTOM one I reported above, none of them had any appearance of success.

I did one more thing, added another lineup in Schedules Direct (on the website), that of "Comcast Tualatin Valley Regional", and attempted to associate with that in NextPVR (thinking that maybe there was something wrong with the Frontier Digital lineup, and all others failed because they weren't registered with SD). That also resulted in "undefined".

Then I thought what the heck, I got no EPG but I have some channels, maybe I can watch some TV (on the tuner above where I had still associated all the channels with "(no listings)"). Select the program, select "Watch" in the dialog, I get one frame of video that looks correct but no moving pictures. Tried a few different channels, same result.

Are there some logs or something I could provide that might shed some light?

If you click on the "Log Files" button shown in one of your screen grabs, that will zip your log files. Also, you may need to zip and upload your NPVR.db3 database located in /Users/Public/NPVR-data/

Indeed. I didn't expect them to be so obvious. Attached are the logs, in case they are interesting.

I don't have /Users because I'm on Ubuntu, where are the databases stored on Linux?

Sadly ... not so much.

It appears that the log rolled over and deleted earlier versions that contained an EPG update attempt (or I have missed something in the many acres of log Smile )

Please recreate the problem and promptly attach the zipped logs to a post telling us the time that you attempted the EPG update. Thanks.

Is there anything we know that resets the logs to cut down on clutter before doing something interesting? Perhaps, stopping and restarting the service?

Attached is a log that should have the attempt to assign the channels, at 23:43 PST. It's the same minute as stamped on the zip filename, I tried the update and then saved the logs directly.

Seems like it might have something to do with:

... but I have no idea what the problem is. I only have one lineup on my account, the lineup that corresponds to my cable provider, which I'm also using daily on my v4 install.
