Maybe try one more clean test no Kodi, no icon downloads so we can see what is going on in the log. Whatever is going one seems unique to you. You only have OTA channels right?

Was typing up a response that it seemed to be working and it crashed again. Lasted about 15 minutes before crashing this time. After that I restarted the service and started up live tv from web interface, no kodi, crashed after a minute. Log attached. Been using this for a couple months without issues so not sure why its all of a sudden so fragile. Are these background activities supposed to lock the database or is this an indication of insufficient hardware? System doesn't seem too taxed, so I dunno.

It still isn't clear what is locking the database or why NextPVR web is loading the channel icons so much but sub needs to reply. Maybe check the NextPVR folder for any hidden sqlite journal and temp files.
On the particular record lock that is causing things to fail, I have asked sub to shorten the timeout on that read and ignore the error but there must be things getting in the way of doing that. Martin

Nothing looks out of place to me when comparing to a backup from a few months ago, but I'm a bit in over my head digging through the files.

Sorry to bump, but in case this helps anyone in the future, I finally traced this issue back to a bad network cable, of all things.
