Hi.  Here is what I experienced.  Log file attached.

Watching live TV with client.  I had paused live tv a few times so I was some minutes behind the broadcast.  A show was recording on the other tuner.  I hit function F1 to bring up the TV guide overlay.  The tv guide appeared OK over the live TV.   I navigated to a future time slot on the same channel and selected a show to record.  Hit enter, then selected quick record.  At that point everything stopped/froze.  Keyboard inoperative.  After a period of time windows offered to quit NPVR or to WAIT longer.  While waiting the client crashed and the desktop was back.

I restarted the client. went back to live tv and repeated my above steps.  client crashed again.

Over to you to investigate for future.  Many thanks for all that you do.  Using the latest windows 10 PRO with all updates applied. 19043.1288

Sorry, but as a very first step I'm going to suggest updating to the current 5.2.2.211017 release. If it still has the problem, I'll advise next steps.

Ok. I will give it a go. I hardly ever use the web interface so I totally missed the availability of the update. There is no alert from what I can see in the client.

Ok update done.  Not sure what to make of the test rerun.  Attempt 1 resulted in NPVR accepting my recording request, attempt 2 resulted in a similar crash to what I reported, attempt 3 and 4 worked fine just like attempt 1.

Maybe I can try again tomorrow and see what it does.  Logs from the updated system attached.

How "future" was it? ie, if the recording was due to start in a couple of minutes, then it might have had to take the live tv tuner to use for the recordings (to start the pre-padding). In theory NextPVR is supposed to return to the menu with a message saying the tuner was needed for a recording, but I haven't tested that specific scenario in a couple of years, and it's totally possible I've broken something with it in that time.

It hasn't worked here in quite some time.  IIRC, the screen just goes directly to the screen saver when the live TV tuner gets reassigned to a recording.

Yes I have noted the missing message in UI client testing too, but I considered it was because of the failure to send a stop command to the client.

@eastvin's thanks for the new logs. The seems to be here

2021-10-18 00:03:44.542 [ERROR][9] Live stream expired without 15 second renewal. (handle:170001)

You seem to be doing maintenance like cancelling and recordings too so I figure all this recording scheduling is blocking the renewals. Crashing would be the decoder not dealing well with the stream that ended .

I found the cause of that popup not appearing, and have fixed for the next build.

This situation a bit different on the UI client. It never actually sees the lease renewal failure, so doesn't have the ability to show these messages. I'll see if there is any alternative way it could be hooked up for this type of client.
