I do not get audio during live tv. Audio works fine when playing video files.

What I found on some channels is I needed to change the audio stream.

While watching a tv channel go into osd screen. For me it's enter key. Then audio settings and see if you have more than 1 audio stream. I need to use 5.1 for my HD channels.

The video quality is also terrible -- very pixelated and blocky. NextPVR live tv video quality is great. Any ideas what is going on there?

Some people on XBMC forums reported that their audio and video problems went away by doing an uninstall/install of XBMC. I did this for Frodo beta 3 and I now have good audio and video for live tv.
