Sorry for my delay, work schedule has been crazy. The extend shows up like a prime unit. But there are no cablecard type of channels or ability. It only has a COAX cable for input only.

From the attached, you can see the tuner with the 105422D3, is my Extend unit. At that time I imported in channels from the Prime unit that I had exported previously. The 101793D5 Normal working HDHR, 10136E6D is the bad unit, I have removed it from HDHR config/setup and unplugged the unit. Guess I don't know how to remove it here. The last unit is 1311B1DD and is a working PRIME unit.
Until I added this unit, the guides were all what I expected. But now this is what is present.

It's not really clear how you've setup your channels, and whether you deleted your old channels from the device, so maybe you should follow earlier my advice about renaming your npvr.db3 file and starting from a new clean database.
As mentioned, you may still your old tuner listed on the Devices screen - this is the way the HDHR drivers work. Just ignore this old device and don't try to setup any channels on it and it won't cause you any problems. If you really want the old device not listed, there was a thread about how to fix it on the SiliconDust forums - from memory it involved removing and reinstalling the HDHR BDA driver.
