I'm sure that previously the channels would match the allocated LCN? Instead they're grouped by multiplex. If there's a way the data on the mux can be decoded to match the allocated channel number instead of manually altering them, which has become an issue when the channel list is all groups when editing alongside the IPTV/VOD.

They still do. The UK doesn't transmit as frequently as other countries and sometimes the scan will miss then. Usually just cancelling and rescanning is enough to pick them up, otherwise there is some frequency wait time setting in config.xml you can use to give it a better chance of seeing the LCN in the stream.

I fell foul of this when I first changed from using Windows Media Centre to using NextPVR: the channels were grouped by multiplex and the channel number was not the proper LCN, despite rescanning several times. I was very unimpressed with NPVR until sub pointed out that in the UK the LCN isn't broadcast as often as in some countries and suggested the config.xml edit.

Since then, I've rescanned many times, as broadcasters change the channel line-up of some multiplexes as minority channels come and go, and I've never had a problem since.

For those people in the UK, an interesting situation has just arisen which shows that it is possible to edit the LCN and name of a channel. ITV3 (archive repeats channel) uses LCN 10. It is a "sub-SD" channel whose picture is 544x576 rather then 720 x 576. A few days ago they started broadcasting a duplicate version in full 720x576, on LCN 788. I was able to swap the LCNs of the low and high def versions (and edit the channel names to distinguish between them), so the new higher-def ITV3 is in the rightful place of LCN 10. I presume at some stage in the future, the LCNs will be swapped by the broadcaster and then the 544x576 version will cease to exist.
