Browse button to select install folder is blank (perhaps it should show three dots) ... plus the dialog box is headed "License Agreement"

Hmm. I see those three dots on the button. What resolution are you running your desktop at?

I've uploaded a new installer to fix this. I also changed the "..." button to a "browse" button.
