Streaming Failed (transcoder exited)

1, I use USB TV tuner 
it works fine normally but once it's messed up 
it doesn't work until I unplug and plug again.
It keeps saying Starting Device. Please Wait... and then Streaming Failed (transcoder exited)

I attached two log files. first log is I downloaded when tuner didn't work
second one is I downloaded after it's back to normal.

2. is there page which I can check ip address who accessed the page?

That error is very generic the issue seems to be signal related there are first time failures on some channels
