With your comment about files being left behind, combined with my earlier comment about the logs not using rolling files on some attempts, it sounds like some attempts to stop live tv aren't stopping, maybe leading to subsequent attempts failing? We'll have to try working out what attempts are causing this.

For example, if you restart everything, then watch a channel and confirm the log shows rolling files, then stop that channel and watch the same channel again, does the log show rolling file second time around, or one big file?

After I stopped I did immediately play the channel and it started a rolling file properly and it deleted it too.

I think it is worthwhile for you to look at the log, maybe something is going off the starting here

Here is it moving from rolling file mode to straight playback. It didn't crash but it stuttered terribly

I accidently changed channels from the mini guide and got one of these non-freeze errors

The channel changed fine and I wouldn't have noticed anything but it did turn to direct streaming mode. This seems to always happen with this

Do you still have ATSC there as well? I'm curious if you can reproduce this on a digital device. Knowing this would help me narrow down the cause.

Sure do, I just set up test, I'll let it go from here. The OP's problem was with an HDPVR.

Yep, and others that seemed to pipe up were using the DCR-2650. Both of these use the 'NPVR Writer' for its rolling file, which is slightly different from the writer used for digital streams.

Here's an atsc hang. Killing the recording service did not put it into direct mode.

Here's a nice new log message
