Using Process Explorer from Sysinternals I see all the ports close when the crash happens.
Intel i5 3.2Ghz Quad Core, Windows 10 Pro 64Bit
12 GB RAM, Gigabit LAN,3TB SATA HD 8MB Cache Dedicated Recording Drive
SiliconDust Prime, PCH A-100 & C-200, Chromecast, FireTV
Raspberry Pi 3 Running OSMC, Samsung Smart TV UN32H5201AF
Cable Provider: Comcast in Oregon, USA

It looks like it's successfully sending the request to the recording service, but maybe the recording service is dying before it can respond.

Yep in process explorer it shows it completely shut down

If you create c:\temp directory, do you get an *.mdmp file created in there when it crashes?

No I don't is there a setting to turn that on.. Its been awhile

No, there is no setting for it. It should create it if the directory exists, but it can only create it in some types of crashes.

If you try running "NRecord.exe RUN" from a command prompt, instead of running the service, does it give any useful message when it crashes?

I'm also not getting anything in the C:\Temp directory. When I ran NRecord.exe from the command prompt, I don't get any additional output in the window, but I do get a Windows popup saying that NRecord Has Stopped Working. It shows the following details:

I've tried reproducing this here, but have been unable to. I don't suppose someone would let me remote into their machine for a few minutes to check it out?

Brett - I put a patch on your machine. Can you see if it helps? It seemed to stop the crash. I couldn't see any video, but I wasn't sure if that was because I was using remote desktop.
