It is the drivers. I fixed the issue by reinstalling the recommended drivers.

Of note, about once a week the Aux3 does not turn on, but rebooting the machine resolves that.

This problem has been around in v4 for some time and was also found to affect V5.

It seems to be triggered by a corruption in the recorded stream. Different softwares handle this corruption better than others but in the case of NextPVR it would jump to FastForward video or freeze (dependant on selected decoders).

I know Sub did some work on this and managed to get a fix for V5 but as far as I'm aware, hasn't retro fitted the fix back into the V4 software.

Yeah, I'm pretty sure it is related to this, and fix in V5.

It wasn't a corruption, but rather the broadcast clock wrap around that happens every few days.

My wife said this just started happening on our recently-new HTPC with v4.2.5 and ATSC recordings. She said it was repeatable and she could avoid it by skipping over the spot in the recording where it started on the first playback. I saved the log from when it last happened, but she deleted the recording before I could tell her to save it.

Does the selected MPEG2 decoder have any impact on this?

If the logs are of interest I can post them.

No, it happens when the timeline wraps around every few days in the broadcast stream. The occasional recording would cover this period, and would have issues skipping.

Good to know, thanks.  I thought she said it was happening in several recordings just recently, but I'll have to confirm the details tomorrow (she's already asleep).  It was a "The Family Feud" game show recording that they were watching when I grabbed the logs tonight and it is likely recorded at the same time every day, if that makes a difference.  It may be they were binge-watching several episode that had the issue.

I tested several problem recordings in V4 and V5 (after Sub added the fix to V5). The jump to FF was repeatable in V4 every time.

In V5 the recording would pause for 1-2 seconds at the problem point then resume normal playback without an issue.

Given V4 is end-of-life I know Sub doesn't want to do work on it so the fix is basically bite the bullet and move to V5.

I would love to go to v5 and get rid of my last Windows box, but I can't get the wife off the PCH clients which are not supported in V5.

I think we may have a different problem anyway.  It seems to be jumping into FF during playback after it jumps a commercial block found by Comskip.  She saved a recording and playback time for me to look at when I get home from work.  I'll give it a try and post logs.
