I have been doing some testing today. Connected up my old USB tuner, and it works first time every time. BlackGold replied to my e-mail, and have sent me the latest drivers which unfortunately didn't help. I did re-check all my cabling, and even connected a splitter so I could run the USB tuner as well as the BG tuner to try and do some signal comparisons. Basically the quality is 100 on both.
I installed another PVR application to see how it handled the BG tuner, and it worked perfectly. I tried reboots and a hibernate, after each, the other PVR could bring the BG tuner online straight away.
Does anyone know if there are any other settings in NPVR around initialising tuners? Timeouts, retries etc?

Do you use both DVB-C and DVB-T? If so do you test both in other apps too with OTA EPG collection? Since installing BDA drivers uses a hack on Windows Server are the other PVR apps you tested using BDA as well?

I only use DVB-T, and I only configured the other PVR app to use the BDA tuner.

NextPVR is using some devices labelled dvbc which might explain issues you are having.

It looked like he has tried scanning the DVB-C device too, which is the only place I can see it using the DVB-C filters

Does unticking "keep digital tuners primed" make any difference?

True but I tried many times so I thought it unusual. The logs for where it was primed go back more than one month like the DVB C scans.

Unticking keeping tuners primed didn't help. The DVB-C tuners are all disabled, so I would have thought that apart from the initial startup, they should be ignored. I have now completely removed them. I did notice today that trying to stream from the NPVR site seems to fail every time until I've restarted the service, but trying through Emby after a couple of attempts it kicks in. I've not checked the logs yet.

It does appear that you are still using the withdrawn Emby plugin, not sure if that is a factor or not.

Also your tuners show mix of primed and not primed, which could confuse things. After ticking unprimed did you restart the server.

After unticking the box, I restarted the service.
I didn't realise the Emby plugin had been withdrawn, just seen on the Emby forum. It was great being able to go to one place for all entertainment stuff... The live stream does seem to work after a couple of retries using Emby, but through the NextPVR web interface, it never starts until I restart the service. I haven't tried to use the NextPVR client yet, I be only just remembered it's still works with V5 on the local box.
