As expected the turning everything off and on again has sorted the problem.
It appears that the system can, on occasion, miss recording multiple repeats on different channels while still successfully recording all non-repeats on multiple channels.
As per my initial post, I'm not particularly looking for a fix, just letting you know this can happen. If no evidence appears in the logs then I've no idea where you could go with this in future.
