For me to check why its not scanning you might need to try setting your mux to 'full ts', then do a 1 minute recording of one of those unknown channels, then make the resulting ts file available to me.

I'm currently using 2 and 20 with BDATwinhanDiseqcMethod = 0. I am not sure what the default settings were - wish I knew since I forgot to back it up. Maybe 5 and 25? I have an old config.xml in which I was using 1 and 10 and BDATwinhanDiseqcMethod = 2 - don't remember how well that worked... :o

Would like to know about the epg as well. Have gbpvr set to use the stream for dish epg, but have not been able to get a single entry to show up in the epg.

If you've been trying the DVB EPG, then you probably need to switch the Schedules Direct or some XMLTV source. Dish Network is not using the standard DVB EPG.
