I'm puzzled as to why you're so puzzled. The first post in this thread has all the patches. I just add them to the list. The hyperlink takes you to the specific post where Sub posted the patched file. You should read Sub's post and some of the posts around it to understand exactly what the particular update is supposed to achieve (hence why I don't link directly to the patched file); my description is just what I understood from a quick read of the thread.
Later on, I will grab the latest versions of every file updated since the original release of the current version of NPVR (2.2.6 in this case) and offer them up in a single .zip file which is attached to the first post (i.e. the cumulative update R1, R2, etc.).
Generally this has been very beneficial, however this time the combination of updates in R2 has had some bad consequences. Primarily skipping is more busted than ever. It seems like everyone with this problem (including me) is just hoping someone else will take the time to provide logs and test cases for Sub so he can fix the problems. On request I did make the R1 version available so that those burned by R2 can get their systems working acceptably again, but that really doesn't help fix anything. So you shouldn't be surprised that the recent handful of patches hasn't done anything to fix the various skipping issues, since apparently no one (again, including me) has done anything to help sub fix it.

Ok, as we appear to have reached the point where sub can no longer provide compatible patches for 2.2.6, I've rolled up the last remaining patches to create "NPVR 2.2.6 Patches Final.zip" now posted in the top post. It includes patches 1-50a (the last 3 were re-numbered; there are no actual new patches since the last update). This will have to hold us until 2.3.x is out.

I'm not at all sure that the majority of users are any better off with 2.2.6 "Final" than they would be by just installing 2.2.6 from sub's installer and then leaving it alone (the installer does include the critical patch #1). Yes, a near-record number of patches have been created for this version, but a large number of them are for isolated cases and minor feature changes and some things have broken along the way. The "R1" patch release might be the sweet spot, and I have linked it at the beginning of the patch list.
At least for this release, I am doubtful that posting these cumulative patch updates are really doing anyone a service. I am running the "Final" file versions myself, but I wouldn't blame anyone who wanted to back-rev. I had previously argued against back-rev'ing, even while providing the earlier files for those who wanted them, on the grounds that running the latest versions and working out the bugs is the only way to make future versions better. At this point though no further changes can be made to 2.2.6, and sub has reported fixing the biggest bug(s) introduced by post-R1 patches for the next release, so feel free to run whichever files make your system work the best today.
We'll have to hope 2.3.x comes soon, and we'll see how things go with that one.

my best so far is all the 'final' patches, but use the unmanaged.dll from R1 instead, and skips are precise and quick...nothing else i've tried works...

+1 this works best for me as well. The R2 unmanaged.dll was a disaster with skipping on my setup.

Final patch would not start livetv automatically, or even at all. I did a reinstall, and patched to R1. All, fine.
Then patched to final, but NPVR would hang.
