The entries in your database are not the same as mine - they're missing the pilot entry. Whether that matters I don't know, but it's something to look at for a start.
You've also renamed BBC One HD to BBC1 HD. That might cause NPVR to miss it on a retune. Doesn't explain BBC HD though...
Your xml for the two BBC HD channels - stored in the CHANNEL_MAPPING table - doesn't contain the <pilot>0</pilot> entry that I listed earlier. That might be because it doesn't matter, or it might be because I don't have the same tuner as you, but you said you'd copied in my tuning info but that suggests perhaps you didn't?
Let's try another tack. Set up a tuner with the attached ini that contains nothing but the single BBC transponder for their HD stuff and then scan and see what result you get? Shutdown the NPVR service beforehand and delete/backup all the logs before restarting it to give us a clean sheet to work from.
Just for sanity's sake, I added the pilot entry (forgot to paste it in from your config earlier) and it made no difference. I also tried reversing the priority of the tuners, in case one of them was either faulty or subtly differently configured. Both changes made no difference. Note that the BBC1 HD channel name (altered so it fits on the TV Listings screen, otherwise it's truncated and you can't distinguish it from BBC One) has been there for a long time, and everything has since worked perfectly - until the recent BBC retune...
And, sadly, after following your instructions, there's still no video, even though both channels are now showing modulation 8PSK in the dialog.

Actually (I doubt this is related) but you've got some pretty weird AutoRefresh settings in there, eg a default rate of 25Hz and a rate of 34Hz for 25fps content? For any UK Live TV you'd want 50Hz.
I'll take a look at the rest of the logs now but it might be time for sub to step up to the challenge.
