I believe I am having the same issue as the original poster. I have upgraded to 4.2.1 (180602).
The error log is showing:
"File cannot be found: C:\Users\Public\NPVR\web\temp\........ts"
Followed by:
"Unexpected error in WriteSafeReadAllLines: System.IO.FileNotFoundException: Could not find file 'C:\Users\Public\NPVR\web\temp\......m3u8'.

Although you didn't provide all the logs this doesn't look quite the same it looks like your server cannot transcode the recording fast enough for live TV play.

I snipped from the time I started streaming to the end. The log was quite large with mostly unrelated entries. Is there some startup data from the beginning that would be helpful?
Can you suggest a monitoring strategy that would determine whether it's processor or hard drive lag?

You can look at the task manager to see how much CPU is being used while you watch Live TV. If it's pegged at close to 90%+, then your processor isn't fast enough.
