I'm wondering if your use of the NVIDIA Audio Decoder is somehow involved in this problem. For comparison sake, your previously supplied NipTuck recording gave both audio and video here using the internal reader, and the following filter list:

It depends on the source. The haali reader is kind of a combined source/demux in one filter. When you use the internal TS reader (GBPVR TS Reader) it uses the Microsoft Demux to do the demultiplexing. Most other third party TS source work this way, for example TSFileSource and HDTVPump both rely on the Microsoft Demux to do the demultiplexing.

Did you look in PVRX2.exe-native.log to confirm that decoder was really being used? I have the feeling it is always using the 'system default' MPEG1 Layer 2 decoder, rather than what you might have selected on the playback tab. ie, I wonder if its still using the nvidia one.
I should add that I can play this via GraphEdit. In this scenario it does not use the mpeg-2 demultiplexer and is seems to be the same as externalTsReader.
