Are you passing the sound through a receiver?

No, through 5.1 onboard sound card. Outputs of card are: Center channel to TV, subwoofer to sub, front speakers to bookshelf speakers, and surround to small satellite speakers.

My app doesn't handle any of the actual video/audio conversion, my app opens the movies through a web browser plugin in .NET. That is the only thing Netflix will allow.

I am pretty sure that if you use ffdshow then it should already be handling the audio and video decoding. I had the same problem and one thing I did that seemed to help tremedously is I went to the ffdshow audio decoder configuration screen, selected output on the left side, under supported output sample formats, selected 24 and 32 bit integers and applied. See if that helps you out, that made a HUGE difference in my audio/soundeffect quality and helped normalize the volume.

Also, if it is only happening on one movie and not all of them, then it is probably a problem with their encoding, so report the problem to Netflix and they will fix it in a couple of days.

Let me know if any of this helps.

can you downgrade your aac to stereo mix? I use AC3 Filter

I have FFDSHOW set so that when I play an MPEG-2 or a vid with AC3 it autoloads FFDSHOW. The Netflix Viewer launches, but FFDSHOW doesn't. So I guess I need to figure out which codec Netflix streaming uses. Will rate app now!

its silverlight... i dont think any codecs have anything to do with it

Silverlight will use internal decoders. It doesn't allow the use of external decoders like ffdshow because they would break the secure playback chain rendering the DRM useless.

Sounds like a Netflix encoder issue then. Let me know what movie it is and I'll test it out and let you know if it is messed up for me too.
