is it impossible to report a position somewhere in the buffer, that way video skipping could be even more accurate?

I have no visibility of exactly where in that buffer the playback is up to, but assuming end of the buffer is pretty close to accurate for video playback (maybe few hundred milliseconds out). With video playback you're never going to get super accurate skipping anyway, since you're not going to see a picture until PCR and iFrame come up in the stream.
