Will NextPVR v4 or 5 work with any of the commercially available IPTV services?  For example:

Cool. What do I need to make it work? If the IPTV service offers 4 device support, could I setup them up on the back end that has a VPN and access them on a client device running kodi via npvr addon? I wonder anyone here has had success with similar iptv services?

If you set ... Max Connections --> 4 ... NextPVR will allow you to, for example, watch one channel and simultaneously record shows on three other channels if your provider allows four simultaneous streams.

There are several ways to watch Live TV and record and watch shows as described at ... https://github.com/sub3/NextPVR/wiki/Other-Clients

Kodi can be used with a NextPVR addon that conforms to the Kodi "design" for PVR addons ... This is available from the standard Kodi repository.

There is also the knewc addon for Kodi ... https://forums.nextpvr.com/showthread.php?tid=50833

NextPVR supports all sources on all clients so you can use IPTV any way that you want.  I suggest that you try this and that and pick a favourite.

I can import the channels and the majority has EPG data. However, there are too many channels that I don't need but I can't get rid of. Is there a file that I can manually edit because the systems tools are not working or I don't know what the heck I am doing. It is almost 10k channels. I tried unchecking the channels I don't want and I tried turning the channels off via the group but they still show up. I currently reverted back to a backup but I really need a clean way to editi the channels.

Assuming you're using the current version, NextPVR v5, then when you do the 'Import M3U', there is a filter drop down at the top of the channel list before you import the channels, where you can select the channel groups you're interested in. ie, use the unselect checkbox at the top to unselect everything, then you might only want to select groups like 'UK Entertainment' or whatever you're interested in. This way you don't end up with all the channels you're not interested in.

NextPVR v4 had much more primitive IPTV support.

I tried that method. I will give it another shot.

This is the filter. I'd start by unticking 'select all' at the top, then only select the few groups you're really interested in. Often these IPTV services have like 20,000+ streams, and users are only really interested in 100 or so channels. (and leaving the other 19,900 enabled is just going to make everything slower etc).
