There's no easy way around that.

If you want port forwarding to work to reach your NextPVR server from outside your LAN I would request your provider put you on a public IP. I was on CGNAT initially and requested a public IP. They said they would but it cost $5 more and had to agree to their terms, so went ahead and paid extra (it used to be free a couple years ago).

If that's not an option you could use a cloud VPN like ZeroTier. It's free and it works well (I used it before). You can check out their website for info on how it works.

When I contacted my ISP about the CGNAT they told me I could only get a static IP if I went to their premium plan; $150!
I was already signed up for the basic plan at $56 which is all I need.

I spent some time working with my VPN provider to get around it, but it just got too frustrating without any results.
I looked at ZeroTier briefly earlier, but didn't get too involved with it at that time.

My goal is for my sons to access my Synology NAS (behind my CGNAT ISP) and log in to a docker version of Jellyfin running there.
If you think that ZeroTier will allow that I'll give it another look.

You shouldn't need a static IP. You would just need a dynamic public IP. Dynamic just means it can change. Then you use a dynamic DNS service (for your router) to provide the IP. But it sounds like they won't help you.

ZeroTier should work for your setup but it would take some learning. Another option might be OpenVPN Cloud Connexa. I haven't used it but I read where others have used it behind CGNAT and it's free for up to 3 networks I think.
