Error 'Recording failed: No video data received'

I have installed NextPVR 5.1.2 on my Windows 10 PC, and I have configured it to use a Hauppauge WinTV-quadHD tuner card. I can scan and watch live TV channels, but I have a problem with recording and playback. Whenever I try to record a program, either manually or with a schedule, the recording fails and I get an error message saying 'Recording failed: No video data received'. I also can't play back any recordings that I made before, as they are all corrupted and show a black screen. I have checked the log files, and they show some errors and warnings, such as 'Failed to open file', 'Failed to write to file', and 'Failed to read from file'. I have attached some screenshots of the recording settings and the log files.

Can anyone please help me fix this problem and enable recording and playback on NextPVR?

First, you are not using the current version of NPVR which is at version 6.
Second, without seeing the log files, there's not much advise to offer.
Please post a copy of the logs telling us the time of a failed recording.
