streaming failed again

I have been happily using next PVR to stream various channels. 

over the last few days its hit-and-miss

If i use windows standard iptv app it works 

i have attached the logs. Its the "italia 1" channel im using as the example

It looks like you are using an URL from a CDN these can never be guaranteed to work for a long time. I'd confirm that you truly are using the same URL for NextPVR as you other app and that any VPN you might need for geo location is working.
