Finally fixed it! For the longest time I was tweaking and trying different video codecs, installed radeon omega drivers, defragged - to no avail, and the constant "hiccups" in playback seemed to be getting worse. Sometimes it would hiccup so bad the playback would freeze and I'd have to hit "Play" again.
I was using the line out on the motherboard audio - realtek ac'97. Usually the first thing I do on a build is disable this, but due to minimal PCI slots - I went with it.
I hit the store yesterday, bought a SB audigy card and haven't had a problem since. Bought a larger case to be able to use the 3rd slot, oh well. I can sleep again.
