I think I am losing my mind. First setting 2.0 in the ffdshow mixer fixed the problem. Then, I played the file a second time and the problem was back. Then, installing the AC3Filter fixed the problem. Second time? Problem was back. I just used the Codec Tweak tool to downmix the output from any codec to 2.0; THAT fixed the problem. But guess what happened the second time I played the file?
Edit: It appears that ANY audio reconfiguration after running the tweak tool overrides that setting. I had saved my AC3Filter settings before attempts. I've now managed to play the file three times with correct 2.0 audio.
However, this brings an annoyance to my attention: What CAN'T I get 2.0 through my TV's speakers and 5.1 (or 7.1, or whatever) through its SPDIF output? That's what my PVR and DVD player do.
