New update, v1.03, is available for download in the wiki and from the second post in this thread.

Do you know why icon view of the video screen creates a second, much larger (higher resolution) icon for some of the videos but not others?

I think that is a problem with NPVR.
I've seen that in other skins as well. At least in older versions of NPVR.

On the other hand, if it DOES work in other skins, I'll have to look into this again (then it may be a size issue).

What happens is the larger preview in the metadata section looks nice for some videos and looks like complete crap for the ones that don't create a larger thumbnail. It's weird how it's only the top row that gets the random higher res thumbs. Nobody else knows anything about what's happening here?

Could you do a printscreen on the problem and show me?
BTW, add the logs as well...
